Library Card Form

• Must be presented each time materials are borrowed.
• Is valuable-please protect it.
• Has a replacement cost of $1.00.
• Parents must sign for children 16 & under.
• You are responsible for all items checkout on your library card, so consider carefully before you loan it to someone else for their use.
• Please report lost cards or a change of address promptly.
• The card you have been issued may be used at any public library in the South Central Library System. This includes libraries in Adams, Columbia, Dane, Green, Portage, Sauk and Wood counties.

For your convenience you can print the form, complete it and bring it to the library along with one piece of legal name verification and current address verification.

Any of the following are acceptable forms of identification:
- drivers license
- utility bill
- mortgage papers
- state identification card
